Stanford chronic disease self-management program comes to Manassas

Mark your calendar to joiMendAHousen Project Mend-A-House and Freedom Aquatic and Fitness Center for a FREE program for people with chronic conditions and their caregivers. The 6 week workshop will be held on the George Mason University’s Manassas Campus and meets once a week for 2.5 hours, beginning on July 15th. A free membership to the Freedom Aquatic Center for the duration of the workshop is included! Space is limited.

“The Freedom Aquatic & Fitness Center is working towards expanding our programming offerings with a focus on health and wellness for community members. The partnership with Project Mend-A-House and the Chronic Disease Self-Management Program will allow us to provide new opportunities to assist patrons with education and support as they work towards making improvements in their health. We look forward to the possibilities that this partnership will provide and the lives that will be impacted in a positive manner through the program,” said Don Jones, PhD, General Manager, Freedom Aquatic and Fitness Center

The evidence-based Chronic Disease Self-Management Program (CDSMP) was developed by the Stanford Medical School. It is designed for people with chronic conditions like asthma, diabetes heart disease, lung disease, cancer, MS, arthritis, COPD, obesity, fibromyalgia, Parkinson’s, depression, pain and other chronic conditions. Caregivers are also welcome. The program is designed to help people live well and better manage their conditions. Sessions include information on:

  • Managing symptoms and dealing with issues like frustration, fatigue, pain and isolation
  • Appropriate medication use and evaluating new treatments
  • Working with your healthcare team
  • Decision making and setting goals
  • Effective problem-solving and communication
  • How to relax & handle difficult emotions
  • Tips for eating well and increasing activity to improve strength, flexibility and endurance

“As a Master Trainer for the CDSMP, I am so excited to finally be able to bring this fantastic, proven program to the Greater Prince William Area! Having worked with the program in neighboring counties for the past three years, I have been frustrated that residents in our area could not participate. That is about to end. Our partnership with the Freedom Aquatic and Fitness Center is a great opportunity to launch this workshop in our area.” stated Jennifer Schock-Bolles, Executive Director of Project Mend-A-House
